foreachpatientandcomparedwiththenationalcostscaleaccordingtothelevelof

socialdisabilityforeachpatient.Thedatawereobtainedfromtheanonymizeddata

oftheGHS[Table1].

matedforthewholehospitalfromthequalityoflifeanddifferencesbetweentheALS

observedandthenationalcostscale.Thequalityofliferesultingfromeachclassof

socialdisabilitywascomparedtoestimatetheadditionalresources(RS)required:RS

=Q1/Qi(withQ1,theaveragequalityoflifeobservedinclass1[nosocialdisabil-

ity]andQi,theaveragequalityoflifeobservedintheotherclassesi,i≠1).Theextra

resourcesrequired(ERS)canbecalculatedastheaverageweightedbythefrequency

observedforeachsocialdisability.Wethusobtain:

Estimationofdifferentialhospitalcostaccordingtothelengthofstay:Thelength

thewholehospital.Itisinterestingtocomparethisresultwiththatobtainedfromthe

qualityoflifemethod.Byanalogywiththepreviousmethod,wehave:RS=ALS1

/ALSi(withALS1:thedifferentialaveragelengthofstayobservedinclass1com-

paredwiththeENC[EchelleNationaledeCoût,nationalcostscale];ALSi:thedif-

ferentialaverageobservedinclassicomparedwiththeENC).ENChasbeentakenas

acomparatorbecauseitisthegoldstandardforlengthofstay(theoreticalreference).

ThecostassociatedwitheachGHMwasevaluatedbythenationaldatabasedon40

participatinghospitals.Thus,thenationalscaleisareferenceintermsoflengthof

staybutnotintermsofcost.UsingtheENCwillguaranteethatanydeviationisnot

duetoadifferenceinthehospitalizedGHMofanysocialclass.Atheoreticaldistribu-

tionofstaysaccordingtodisabilityclasscanbeobtainedbyapplyingthedistribution

ofdisabledpatientstothetotalnumberofstays.Anumberofextradayscorresponds

toeachdisabilityclass,exceptforpatientsinthereferenceclass1.Foreachdisability

class,thenumberofextradaysobservedisthenmultipliedbythenumberoftheoreti-

calstaystoobtainthenumberofadditionaldaysresultingfromsocialdisability.The

averagedifferentialobservedcanbecomparedwiththeENCforeachpatientandfor

eachclassofdisability.
