Costsandﬁnancesinhospitals

In2008,thechallengeinFranceandothersuchcountriesistotakeintoaccount

thissocialdimensioninthereformedsystembudget.TheGHSsystemcanneither

ﬁnanciallycoverthepatients,personalsocialdimensionsnordetermineaspeciﬁc

budgetnecessaryinresponsetotheseparticularneeds,whicharenotmainlymedi-

callybased.ThereiscurrentlynoaidfromtheGHStariffs13,14,15totakeﬁnancial

accountofthepersonalsocialdimensionofapatient16ortoallowadiscretionaryor

speciﬁcbudgettomeettheseparticularneeds.

Ouraimistodesignatooltomeasuresocialdisability(notexclusivelyitsper-

ception)ofpatientsinapublichospitaltolearn:1)thedisabilitydistributionand

particularneedslinkedtothesocialdisability,aswellasthepatients,medicalneeds;

2)whetherornottheseneedsmobilizeadditionalfundingresources;3)theextrare-

sourcesthatcanbecoveredbythepublichospitalsothatitfulﬁlsitspublicservice

mission.TheGHSassociatedwithsocialdisabilityindicators

betterknowledgeonpatientsadmittedtoapublichospitalandontheaveragelength

ofstayandnumberofadditionalhospitaldays.Ourobjectiveistoproposeamethod

ofestimatingpatients,socialdisabilityinordertohaveitsmeasurementandtoinclude

thecostsnotfundedbypublicauthoritiesandtheadditionalbudgettobeallocated.

TheFrenchcasestudiedhereshouldbeanexampleforallEuropeancountries

whichmeetthisparticularproblemofdealingwithlongerhospitalstaysresulting

frompersonalsocialdimensionsofapatient.
